The proposed services and fees are based on COBRA
Administration for the Health, Dental, and Health
Flexible Spending Account Plans.
Services
- Takeover Coordination with employer to review
takeover and administrative procedures.
- Processing and Adjudication of COBRA Elections
- Coupon Booklets for Monthly Statements / Collections
Processing
- Grace Reminder Letters
- Consolidated premium payment for each plan sent to the
employer on a monthly basis (includes add-on % for
administration)
- Account Reports sent to employer on a monthly basis
- Administer Dependent Qualifying Events & Disability
Extensions
- Managing of all COBRA participant calls and disputes
- Document revisions as required by law
- Quarterly review meetings as needed
Fees
Monthly Base Fee (for groups under 300 employees)
$ 75.00
Newly Covered Participant Notification
(notice is sent separately to employee and spouse):
$ 3.50 per notice
Qualifying Event Notification: $ 8.00 per notice
Takeover/Set-up Fee (one time only) $ 300.00+ $
10.00 per each current COBRA participant
