Medical

WE have the option of offering quotes from some of the top individual carriers including Blue Cross Blue Shield, John Alden Insurance, Unicare and Golden Rule and Fortis Benefits. We can also provide you with International Travel Insurance as well as Short Term Medical Plans (up to six months).

WHAT if we can provide you with a variety of medical options? You can pick and choose office copayment amount and deductibles to fit your individual medical needs. We also offer options of networks you can choose. New plans are available offering Catastrophic Hospitalization plans to PPO plans. In addition, you can purchase Medicare supplemental policies.

Dental

WE can offer a choice of three plans with rates from $7 to $17.90 per month for an individual depending on the plan selected. We also offer vision coverage for an additional $2 per month. For individual and/or family coverage, you can go online to Dental Select and click on Groups or Associations, then click on Texas, then choose SBU Texas and follow the instructions to enroll online.

Life

NOW you can apply for Individual Term Life insurance.

GROUP COVERAGE

Medical

WE have the option of offering quotes from some of the top group carriers including Blue Cross Blue Shield, John Alden Insurance, Unicare, United HealthCare, Pacific Life, Aetna, Amil, Humana, Guardian and Principal, among others. We can also provide any of your employees that travel abroad with International Travel Insurance.

WHAT if we can provide you with a variety of medical options? We can offer either HMO or PPO plans with Coinsurance amounts ranging from 90/70 to 70/50 plans. You can also choose the office copays and deductibles you would like to offer your employees. We also offer options of networks you can choose. In addition, for your employees or dependents on Medicare, you can purchase Medicare supplemental coverage at a reduced cost.

DOES your current medical plan cover maternity, acupuncture or chiropractic services? It could!

Dental

WE have several options of plans that we offer through Global Benefit Solutions. Some of our plans are simply discount plans while others are full indemnity plans with rich benefits. A few of our plans utilize their own network of dentists while others offer benefits to your preferred dentist whether or not he is in a network. These plans can be set up either as employer paid or voluntary. Through the SBU, you can purchase your Dental insurance at discounted rated from Dental Select. Rates vary depending on the plan selected with rates from $6.65 to $17.54 for Employee Only coverage.

Life, Long Term Disability, Short Term Disability, Voluntary Life

NOW you can purchase these plans at 10% off of the standard rates and the plans are Guarantee Issue, up to certain limits, with no health questions asked!

401(k)

Global Benefit Solutions along with Standard Insurance can set up a 401(k) plan to meet your company’s needs. Standard provides an excellent range of products as well as service.

Section 125 – Cafeteria Plans, COBRA Administration, Transportation Fringe Benefit Plans

We can easily set up any of these plans with your company to facilitate your Benefits Administrator in handling your insurance. Our staff is knowledgeable in implementing these services.

AFLAC Personal Accident Expense

This plan is designed to help cover the expenses associated with an accidental injury. It pays you directly, unless you assign benefits, regardless of any other insurance you may have. Benefits include, among others:

• Accidental Hospital Confinement
• Accidental-Death and -Dismemberment
• Plus many others

AFLAC Personal Cancer Protector

When a covered individual is diagnosed with cancer, this policy provided benefits for hospital confinement, radiation and chemotherapy, and surgery, among others. In addition, AFLAC will pay a First-Occurrence Benefit when a covered individual is first diagnosed with internal cancer. Benefits may include:

• Hospital Confinement
• Radiation and Chemotherapy
• National Cancer Institiue (NCI) Evaluation/Consultation
