The debit card from MasterCard allows you to pay for medical goods and services anywhere MasterCard is accepted. The money comes right out of your account, so you don't have to wait to get reimbursed by a TPA or an insurance company.

Helpful Hints… Tips on using your Debit Card
ATM transactions are PIN based (you must use your PIN to withdraw money). Point-of-sale transactions are either PIN based or signature-based. PIN based transactions are $1.50 per transaction while signature-based transactions are free.
We have found that in some cases the merchant (usually pharmacies) will ask you to use your PIN (personal identification number) when you present your debit card to make a purchase. They may also ask if your card is a debit card or credit card. Currently, we issue all debit cards with a PIN, so you can either use the PIN when asked or just have the merchant swipe your card as if you didn't have a PIN (signature-based). Now, if you were not issued a PIN or can't remember your PIN, have the merchant swipe your card as "credit" (even though your card is not a credit card, it can be swiped as one) and then you simply sign the receipt. In either case, the money will still come out of your account.
Use it like a checkbook – When you use your debit card, the money comes right out of your account.
Credit Card Convenience - Use your debit card for healthcare related purchases wherever MasterCard is accepted.
Records – You will receive your account statement detailing the purchase amount and the place of purchase to help you with your tax records.
Remember, this card should only be used for medical purchases or medical services. (For a list of the qualified expenses go to page 4 of the IRS Publication 502.) The daily purchase limit is $1,000, and you may call HSA Bank during business hours at 920-565-5252 or 800-357-6246 to have this limit temporarily raised for larger transactions. NOTE: Allow 2 business days for limit increases.
Debit Card Renewal - Your debit card renews automatically every two years. The card renewal fee is $3 per card. To cancel one or both of your debit cards prior to renewal, please contact our Personal Bankers during business hours at 920-565-5252 (or 800-357-6246) or via e-mail at askus@msabank.com (NOTE: Do not send private information such as account numbers or SSN via email). You would need to contact us at least one full month prior to the renewal date. To determine the renewal date, please look at the "valid thru" date on the face of the card for the month and year (ex: 10/02). In this example, the card renews in October 2002, so you would need to contact us on or before August 31, 2002 to cancel the card renewal.

Withdrawals from your account for non-medical reasons must be made by sending to HSA Bank a completed Withdrawal form for a fee of $4. The bank will then issue you a check.
LOST OR STOLEN CARDS
Lost Card fee of $6.00 per card. HSA Bank reserves the right to cancel your debit card access at any time. You must return your debit card(s) to HSA Bank upon demand.
ORDER INFORMATION
If you are opening an HSA with us, please request the debit card(s) you require on the HSA Application and Eligibility Form.
The initial cards are free and you may order two. IRS regulations require MSA and HSA accounts to be "owned" by a single individual; however, we can provide you a debit card in your spouse's name (again, the account belongs to one individual).
